Job summary

CarePlus New Jersey seeks a Family Intervention Specialist for Region 5. This full-time, on-site with hybrid schedule role travels to Passaic, Sussex, Warren, Hunterdon, and Morris counties.

You will provide individualized family sessions in home and community settings, manage a caseload of about five families, and conduct three face-to-face sessions weekly to support and strengthen families.

Qualifications

• Master's Degree in social work, psychology, or related field.
• Bachelor's degree in social services with one-year related experience in counseling or case management
• A valid driver's license must be insurable by the agency's automobile insurance.

Responsibilities

• Provide individualized family sessions in the home and in the community.
• Carry a small caseload of five (5) families.
• Meet three times, face-to-face with families to provide support and strengthen family unit.
• Attend 3 required meetings per week for supervision, consultation, and development.
• Complete documentation including treatment plans and session notes.

Travel Required as clients are serviced in Passaic, Sussex, Warren, Hunterdon, Morris counties. FT 40 hours On-site and In-community with Hybrid Schedule Salary: $50,000- $54,500 + $2,000 on-call

Job Summary: As an Intercept Specialist, you will provide individualized family sessions in the home and community settings in order to safety maintain the youth in the home. Working with a case load of about 5 families, you will provide an average of three (3) face-to-face sessions with the family a week in an effort to divert youth from out-of-home placements and to help strengthen the family unit.

What You Will Do:

• Provide individualized family sessions in the home and in the community
• Carry a small caseload of five (5) families
• Meet, on average, three times, face-to-face with the families to provide support and strengthen family unit.
• Attend 3 required meetings per week (group supervision, clinical consultation, and individual development) for program model adherence, case conceptualization, and professional development
• Complete all documentation related to families, including bi-weekly treatment plans, weekly session notes, and ongoing clinical assessments.
• Advocate for the families in any involved system they are part of, such as CP&P and, with the purpose of gathering information specific to intervention development.
• Work closely with DCP&P staff to assist families in successfully addressing needs and strengthening the family system.
• Provide afterhours support to families using a rotating on-call schedule and respond in-person to some crisis situations, as deemed necessary.
