Job summary

WellLife Network in New York is seeking a Family/Housing Specialist to provide support and service linkages to tenants residing in the residence. The role includes intake assessments, service plan development, and coordination of services in line with NYC Department of Health and Mental Hygiene standards.

The position emphasizes collaboration with community agencies, documentation, and crisis intervention, with a focus on the children in the household and the safety of tenants.

Qualifications

• Bachelor's Degree with a minimum of 1 year working with the homeless, families with children, young adults and/or mentally ill chemical abusers.
• Associate’s Degree with 3+ years working with special populations.

Responsibilities

• Provide direct care services to tenants and support daily living goals.
• Coordinate services, document case notes, and monitor progress.
• Establish and maintain relationships with community agencies and facilities.
• Assist with evacuation planning and tenant safety.
• Maintain tenant files and participate in meetings/trainings.
