Job summary

Qualifications

• Bachelor’s degree in a relevant field and one year of related experience.
• Completion of Evidence-based home visitation training (PAT).
• Proficient use of technology for communication and data collection.
• Must obtain Infant Mental Health endorsement within the first year.

Responsibilities

• Provide educational support and counseling to parents with fidelity to PAT model.
• Deliver all PAT components using the Foundational Curriculum and QA guidelines.
• Maintain caseload and conduct home visits one to two times per month.
• Refer families to community resources and coordinate access to services.
• Plan and participate in educational parent group activities and outreach events.
• Complete PAT training and document required data and reports.
