Responsibilities

• Lead and facilitate team meetings for the young person and family.
• Provide care coordination within the family’s home, community, or preferred location.
• Maintain documentation to access and maintain OhioRISE benefits.
• Collaborate with service providers to meet goals and outcomes.
• Adhere to ISBH policies, confidentiality, and reporting requirements.
• Travel within the service area and participate in trainings.
