Integrated Services for Behavioral Health is seeking an OhioRISE Care Coordinator to build and lead a family-centered care team, coordinating services in homes and communities. You will work with families to identify goals and connect with partners while maintaining HIPAA-compliant documentation.

The role emphasizes collaboration, strong communication, and travel within the service area to support youth, with opportunities for professional development and competitive benefits.

Qualificações

• Experience providing services to children and families in behavioral health or related public services.
• Ability to communicate effectively in verbal and written forms.
• Ability to use personal or company vehicle for in-home/community services.
• Knowledge of Medicaid billing and HIPAA requirements.

Responsabilidades

• Lead and facilitate youth and family team meetings to support goals.
• Provide care coordination in homes, schools, and communities.
