New View Alliance is seeking a High Fidelity Wraparound Care Coordinator in Buffalo, NY. This role involves utilizing the Wraparound Model to assist families with complex needs, providing tailored community-based services for children and youth.

The ideal candidate should have a Bachelor's or Master's degree in a human services field, alongside relevant experience. This position comes with a full-time schedule and offers a robust benefits package, including medical, dental, and 33 days of PTO annually.

Qualifications

• Bachelor’s Degree in a human services related field with 1 year of experience.
• Master’s Degree in a human services related field with 2 years of experience.
• Valid New York State driver’s license and reliable transportation.

Responsabilités

• Maintain a case load of up to 12 families providing direct contact.
• Organize and facilitate all Child and Family Team Meetings.
• Communicate and collaborate with necessary providers and supports.

Ability to work independently and be a team player

Possess assessment skills and establish relationships

Formation
