Job summary

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment Specialist to deliver in-home and community-based services for children and families. The role focuses on trauma-informed, evidence-based care and collaboration with a multidisciplinary team.

Certification is required within one year of hire. The organization emphasizes compassionate, client-centered care and supports professional growth through structured trainings and an engaged team.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Masters in human services field + 1 year experience.
• Bachelor's in human services field + 2 years experience.
• Bachelor's in non-human services field + 4 years experience.
• Current NC license or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver's license and access to a fully insured, private form of transportation.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model, setting goals and building skills.
• Coordinate with medical, behavioral health, school, and community partners to ensure whole-person care.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain documentation in the Electronic Health Record.
• Participate in Quality Assurance activities to strengthen programs.
