Job summary

Pinnacle Family Services, part of Health Connect America, is seeking qualified clinicians to deliver Family Centered Treatment in home and community settings throughout North Carolina. You will develop and maintain individualized Service Plans, conduct assessments, provide interventions, and coordinate with medical, school, and community partners to ensure integrated support.

Qualifications

• Master's degree in Counseling, Social Work, or a related field.
• Must meet qualifications as a Qualified Professional with experience serving children and adolescents with a primary mental health diagnosis.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model to support lasting change.
• Develop and maintain individualized Service Plans with the Treatment Team.
• Conduct assessments and provide therapeutic interventions aligned with treatment goals.
• Coordinate care with medical, behavioral health, school, and community partners.
• Support families in setting goals, building skills, and accessing resources.
• Develop discharge and aftercare plans to promote long-term stability.
• Maintain timely clinical documentation in the Electronic Health Record and participate in QA.
