Qualifications

• Valid driver’s license and reliable vehicle required.
• Ability to work in home/community settings and travel as needed.

Responsibilities

• Lead team meetings with the young person, family, and service providers.
• Coordinate care planning in home, school, and community settings.
• Maintain required documentation and ensure HIPAA compliance.
• Travel within the assigned service area and participate in trainings.
• Advocate for family voice and choice in all coordinated services.
