Mohawk Valley Community Action Agency (MVCAA) is seeking a Family Specialist to provide case management for families in Oneida, Herkimer and Madison counties. The role emphasizes supporting families toward self-sufficiency, with trauma-informed care and strong collaboration with community resources.

MVCAA offers a generous benefits package; full-time position in the Family Resources department. Will require travel across counties and detailed documentation using agency systems.

Qualifications

• AA/AAS in human services or related field preferred; may be obtained within 18 months of hire.
• Experience in home visiting, group facilitation and direct support preferred.
• Must be able to lift up to 50 pounds.
• Proficiency with Microsoft Office, internet, email, database entry and Zoom.
• Ability to relate to people of varied backgrounds and travel across 3 counties.
• Knowledge of local referral sources and community resources.
• Ability to work with minimal supervision and maintain detailed accountability.
• Background check, fingerprint clearance, physical exam and TB test required.

Responsabilités

• Provide case management services for enrolled families at risk.
• Maintain detailed documentation in agency database (CAPTAIN).
• Identify barriers and outline action plans with families.
• Assist families in setting and monitoring goals.
• Incorporate Trauma Informed Care in service delivery.
• Maintain regular contact with families and update files.
• Coordinate referrals with area service providers.
• Refer families to resources and advocate on their behalf.
• Facilitate family support groups and educational opportunities.
• Transport families with agency vehicle when needed.
• Participate in staff meetings and trainings.
• Collaborate with other programs to achieve family goals.
• Operate with sensitivity to poverty and diversity.
