Position Details
- Title: Case Manager I
- Type: Non-Exempt, Full-Time
- Reports to: Program Manager
- Location: Center for Women and Children
- Completion Date: February 2024
Position Summary
The Case Manager provides care for Wheeler Mission guests through assessment, goal setting, and resource identification to help achieve stability.
Key Responsibilities
- Assess guests' resources, talents, skills, and interests.
- Develop and monitor personalized care plans.
- Advise and advocate for guests.
- Identify needs, provide information, and make referrals.
- Lead crisis interventions as needed.
- Monitor guest progress and ensure program compliance.
- Facilitate small groups and assist guests in establishing a foundation in Christ.
- Coordinate services with other agencies and maintain accurate records.
- Participate in multidisciplinary case conferences.
- Practice healthy work/life balance through prayer, study, and community involvement.
Qualifications
- High School Diploma or HSE
- 1-2 years social service/ministry experience, including counseling, mentoring, or discipling
- Experience working with individuals facing substance abuse, mental health issues, trauma, domestic violence, or homelessness
- Proficiency in Microsoft Office, Resident Database, and Paycom
- Knowledge of community resources for housing and employment
- Understanding of trauma-informed care and de-escalation techniques
- Belief in the inerrancy of the Holy Bible and alignment with Wheeler Mission values
- Valid driver’s license and insurable driving record
- Ability to lift up to 20 pounds and perform duties requiring standing, sitting, and driving
- Willingness to work some evenings and weekends
- Background check and drug screening required
Physical and Other Requirements
- Ability to walk, ascend stairs, and operate office equipment
- Maintain confidentiality and adhere to policies
