Job summary

Elder Services of Cape Cod and the Islands is seeking a Family Caregiver Support Specialist in South Dennis, MA to respond to information requests from prospective consumers and agencies about the Family Caregiver Support Program and Medicare’s GUIDE Program.

The role includes in-home visits, telephone assessments, researching resources, caregiver support groups, documentation, and community outreach. A valid MA driver’s license and reliable car are required.

Qualifications

• Ability to collaborate, take initiative, provide/receive feedback.
• Ability to communicate and work with the elderly and community resources serving the elderly population.
• Ability to work independently and professionally.
• Valid MA driver's license and car required.

Responsibilities

• Respond to requests for information from prospective consumers and community agencies regarding the Family Caregiver Support Program and Medicare’s GUIDE Program.
• Assist consumers within the parameters of the MFCSP and Medicare’s GUIDE Program as a Care Navigator.
• Includes telephone calls, in-home visits and telephone assessments, researching resources and caregiver topics.
• Assistance with caregiver support groups and workshops, managing computer documentation and participating in community outreach.
