Job Overview

The Family Care Manager (FCM) reports to the Assistant Director of Outpatient and is responsible for assessing the social and psychosocial needs of child and adolescent patients and their families who are experiencing complex psychiatric challenges. The FCM plays a key role in coordinating care and managing transitions within the Ohana system of care, including admissions, discharges, and movement between levels of treatment.

Key Responsibilities

• Serve as central navigator for families, helping patients and caregivers understand and address factors that may impact behavioral health (legal, educational, financial, occupational, transportation, insurance-related).
• Facilitate referrals to community-based services and maintain knowledge of local and regional resources.
• Build collaborative relationships with external providers and organizations.
• Provide psychoeducation through individual support, groups, and workshops.
• Collaborate closely with therapists, psychiatrists, and behavioral health professionals; participate in clinical meetings; advocate for family needs from a social work perspective.
• Perform additional duties as assigned to support high‑quality patient care.

Qualifications & Experience

• Minimum 3 years’ experience working with children, adolescents, or families and knowledge of critical local and regional resources (educational system, legal resources, juvenile justice system, community services, public insurance).
• Master’s degree in Social Work required.
• Effective 10/16/2023 and Associate registration with the BBS at the time of application required.
• American Heart Association Healthcare Provider BLS certification required.
• Will complete mandatory 5150 training by Monterey County and obtain 5150 Professional staff authorization yearly.

Work Hours & Compensation

Position Type: Regular. Salary: $49.77 to $66.56 per hour, based on years of applicable experience. Shift differentials: Evening – $3.39, Night – $5.09.
