Job summary

A Place At Home - Fairfield South in Norwalk, CT seeks a compassionate Fall Prevention and Companion Caregiver to support clients aging in place. You will assist with daily activities, monitor safety, and help clients regain independence per the care plan.

Work with the care team to ensure safe transfers, provide companionship, document care, and uphold dignity and confidentiality. CNA/HHA background and CPR/First Aid certifications are valued; a valid driver's license preferred.

Qualifications

• CNA or HHA or equivalent caregiving experience preferred.
• Current certifications in First Aid, CPR, and TB test (or willingness to obtain).
• Successful completion of background check and drug screening, with random checks thereafter.
• Physically able to perform duties, including safe lifting and mobility assistance.

Responsibilities

• Provide personal care assistance per care plan.
• Assist with safe transfers, walking, and repositioning to prevent falls and promote healing.
• Monitor and report changes in the clients condition to the supervisor.
• Support medication reminders (non-medical, per care plan).
• Prepare light meals and snacks according to dietary needs and assist with feeding if necessary.
• Help with light housekeeping, laundry, and organization to maintain a clean, safe environment.
• Provide companionship and reassurance during the recovery process.
• Document care provided and communicate with the care team to ensure continuity.

Position Overview

The Fall Prevention and Companion Care Caregiver provides assistance and supervision to clients aging in place at home. This role focuses on ensuring a safe recovery environment, offering personal support, and helping clients regain independence while following their care plan.

Essential Functions

• Provide personal care assistance (bathing, dressing, grooming, toileting, mobility support) as directed in the care plan.
• Assist with safe transfers, walking, and repositioning to prevent falls and promote healing.
• Monitor and report changes in the client's physical or mental condition to the supervisor.
• Support medication reminders (non-medical, per care plan).
• Prepare light meals and snacks according to dietary needs and assist with feeding if necessary.
• Help with light housekeeping, laundry, and organization to maintain a clean, safe environment.
• Provide companionship and reassurance during the recovery process.
• Document care provided and communicate with the care team to ensure continuity.
