Job summary

A Place At Home - Fairfield South is seeking a dedicated Fall Prevention and Companion Caregiver to assist clients aging in place. You will provide non-medical personal care, support safe transfers, and help patients regain independence while following a care plan.

Responsibilities include monitoring condition changes, reminding about medications, light meals, and light housekeeping. You will document care and communicate with the care team to ensure continuity and high-quality care.

Qualifications

• CNA or HHA credentials preferred and able to provide non-medical support per care plan.
• Current First Aid, CPR, and TB test certifications or willingness to obtain.
• Successful completion of background check and drug screening, with random checks thereafter.
• Physically able to perform duties including safe lifting and mobility assistance.

Responsibilities

• Provide personal care assistance (bathing, dressing, grooming, toileting, mobility support) per care plan.
• Assist with transfers, walking, and repositioning to prevent falls.
• Monitor and report changes in client condition to supervisor.
• Support medication reminders (non-medical) per care plan.
• Prepare light meals and assist with feeding as needed.
• Help with light housekeeping, laundry, and organization.
• Provide companionship and reassurance during recovery.
• Document care and communicate with care team for continuity.
• Respect client dignity, privacy, and confidentiality.
