Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
The FACT Assistant Team Leader assists the FACT Team Leader in administering and directing the Florida Assertive Community Treatment (FACT) Program, a clinical team that directly provides treatment, rehabilitation, and support services to persons with serious mental illness. The role promotes recovery, empowerment, self-determination, and decision-making for each FACT participant.
The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life.
FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers.
Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; team members also use personal vehicles as needed and are eligible for mileage reimbursement.
The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities.
This position is a Full Time position (rotating weekends, on-call, and holidays).
