Position Overview
The Facilitated Intake Team (FIT) Social Worker conducts comprehensive intake assessments for clients entering the Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS) clinics under the Certified Community Behavioral Health Clinic (CCBHC) umbrella. The goal is to assess clients' needs and recommend appropriate treatments and support services. This position is vital to the initial client experience, requiring strong engagement skills and the ability to gather essential clinical history.
Key Responsibilities
- Review demographic and clinical information for accuracy before the intake process
- Assess social determinants of health, including mental health, substance use, financial, housing, legal, and medical needs
- Complete 6-8 intakes daily
- Present clinical formulations during team meetings
- Recommend treatment modalities based on clinical data
- Conduct crisis evaluations and stabilization interventions
- Make referrals within CCBHC and the community as needed
- Assess and prioritize high-risk cases, including suicidality, psychosis, abuse, and domestic violence
- Understand support services for accurate client referrals
- Collaborate with partner programs for seamless care access
- Participate in evidence-based training to strengthen diagnostic skills (e.g., SBIRT, MI)
- Provide resources sensitive to clients' diverse backgrounds, considering intersectionality
- Complete intake documentation within 48 hours electronically
- Perform other duties as assigned

Educational/Training Requirements
- NY State license in clinical social work (LCSW) or LMHC with diagnostic privileges
Experience and Language Preferences
- Minimum 3 years of clinical experience
- Professional and compassionate client interaction experience
- Bilingual (Spanish/English) preferred
- Experience with evidence-based models preferred
Computer Skills
- Proficient with electronic health records
- Microsoft Office Suite
- Experience with video platforms (Zoom, MS Teams)
Manual Dexterity and Visual Skills
- Ability to read documents and perform data entry
- Limited manual dexterity and hand-eye coordination required
Work Environment and Physical Demands
- In-person role
- Schedule developed with Deputy Program Director, based on program needs
- Accessible offices per ADA standards
