PURPOSE:

The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high-quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person-centered approach to help individuals and their families develop skills and resources to improve overall functioning, instill hope, and strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.

All Staten Island programs are allotted a $4,420 commuter differential that is applicable to anyone who currently works or is hired to work on Staten Island and doesn’t live there, in addition to the base salary or hourly wage.

POSITION OVERVIEW:

The Facilitated Intake Team (FIT) Social Worker provides comprehensive intake assessments for clients entering the Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS) clinics under the Certified Community Behavioral Health Clinic (CCBHC) umbrella, with the goal of assessing their needs and making appropriate recommendations for treatment and linkages to supportive services. The FIT Social Worker plays an integral role in the client’s initial experience, requiring good engagement skills and the ability to elicit important clinical history from clients.

KEY ESSENTIAL FUNCTIONS:

  1. Review demographic and clinical information prior to engaging in the intake process to ensure accuracy.
  2. Provide a comprehensive assessment of clients’ social determinants of health, including mental health, substance use disorders, financial, housing, legal, and medical needs.
  3. Complete between 6-8 intakes per day.
  4. Present a clinical formulation clearly during disposition team meetings.
  5. Make treatment modality recommendations based on clinical information obtained.
  6. Conduct crisis evaluations and provide crisis stabilization interventions.
  7. Make appropriate referrals for various levels of care within the CCBHC and in the community.
  8. Assess for risk and prioritize cases deemed high risk, including suicide ideation/attempts, symptoms of psychosis, abuse, domestic violence, etc.
  9. Develop an understanding of support services for accurate referral.
  10. Collaborate with partner programs/services for smooth access and connection to care.
  11. Engage in evidence-based training to develop diagnostic skills (e.g., SBIRT, Motivational Interviewing).
  12. Provide resources addressing the diverse needs of clients, considering intersectionality.
  13. Ensure timely completion (within 48 hours) of intake documents in the electronic health record.
  14. Other duties as assigned.

EDUCATIONAL/TRAINING REQUIRED:

  • NYS license in clinical social work (LCSW) or LMHC-DP with diagnostic privileges

EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:

  • Minimum 3 years’ clinical experience
  • Experience interacting with clients professionally and compassionately
  • Bilingual (Spanish/English) preferred but not required
  • Experience with evidence-based models preferred

COMPUTER SKILLS REQUIRED:

  • Proficient with electronic health records
  • Microsoft Office Suite
  • Comfortable with video platforms (Zoom, MS Teams)

VISUAL AND MANUAL DEXTERITY:

  • Ability to read documents for analysis
  • Perform data entry into computer programs

WORK ENVIRONMENT/PHYSICAL EFFORT:

  • Hybrid work setting
  • Schedule developed with Deputy Program Director based on program needs
  • Accessible offices per ADA standards
  • Primarily sitting (85%) with some standing (15%)
