Job Summary: As an External Coordinator at Comfort Keepers, you’ll ensure continuity of care for clients across Hazleton, Pottsville, Selinsgrove, Cleona, and Bloomsburg. Collaborate with our Support Team to coordinate in-home services, manage client care resources, and connect with staff, families, community providers, and third-party payers.
Essential Functions:
Handle client inquiries, qualify leads, and schedule visits
Assess client and caregiver needs, ensuring safety and quality care
Coordinate shifts, resolve scheduling issues, and conduct regular visits
Facilitate caregiver and family training
Maintain compliance through documentation and care monitoring
Serve as a vital link between clients, providers, and the Client Services team
Participate in on-call rotations and management meetings
Complete administrative tasks and drive care improvements
Qualifications:
Proficiency in Microsoft Office and related software
Excellent communication, organizational, and problem-solving skills
Strong sales abilities and business acumen
