Job summary

Complete Care – Essex County, NJ is seeking an experienced External Case Manager to support our skilled nursing facilities across Essex County, New Jersey. The role focuses on building strong hospital and partner relationships, coordinating patient admissions and transitions, and supporting census growth.

The External Case Manager will serve as a liaison between Complete Care facilities and external healthcare partners, identifying opportunities to strengthen referrals and drive admissions.

Qualifications

• Experience in healthcare, case management, hospital relations, admissions, business development, or a related field.
• Knowledge of skilled nursing, post-acute care, or hospital discharge processes strongly preferred.
• Strong communication, relationship-building, and organizational skills.
• Ability to work independently while collaborating with facility and regional leadership.
• Must be comfortable traveling throughout Essex County and visiting referral partners.

Responsibilities
