Job summary

Complete Care is seeking an experienced External Case Manager to support our skilled nursing facilities across Essex County, New Jersey. The role focuses on building relationships with hospitals, providers, and community partners to coordinate transitions of care and support census growth.

You will travel to referral sources, advocate for patients, and collaborate with facility leaders to ensure smooth admissions and discharge planning while maintaining high professional standards.

Qualifications

This role is responsible for building strong relationships with hospitals, healthcare providers, and community partners while helping coordinate appropriate transitions of care and supporting census growth.

Responsibilities

• Represent Complete Care professionally within the healthcare community.
• Track referral activity and maintain accurate records of relationships and opportunities.

Qualifications

• Experience in healthcare, case management, hospital relations, admissions, business development, or a related field.
• Knowledge of skilled nursing, post-acute care, or hospital discharge processes strongly preferred.
• Strong communication, relationship-building, and organizational skills.
• Ability to work independently while collaborating effectively with facility and regional leadership.
• Must be comfortable traveling throughout Essex County and visiting referral partners.
