Extended Family Home (Shared Living Provider)

Prime Home is seeking compassionate and dedicated individuals to become Shared Living Providers (SLPs) by opening their homes to support an individual with an intellectual or developmental disability.

As a Shared Living Provider, you will offer a safe, supportive, and nurturing home environment while helping an individual develop daily living skills, increase independence, and achieve personal goals. This is a unique opportunity to build a meaningful relationship and make a lasting impact in someone's life.

Position Overview

This is a subcontractor, work-from-home opportunity where services are provided within your own residence. Applicants must have an available bedroom and a genuine commitment to supporting individuals with disabilities.

Individuals receiving services have a variety of support needs, ranging from behavioral challenges to complex medical needs. Prime Home is looking for providers who are open to serving individuals across a broad spectrum of needs and who are committed to promoting dignity, respect, and independence.

Prime Home is specifically seeking applicants who are willing to support individuals who may display challenging or maladaptive behaviors, including:

• Elopement (leaving supervision or running away)
• Physical aggression
• Verbal aggression
• Property destruction
• Sexually inappropriate behaviors
• Other behaviors that may require enhanced support and supervision

What Prime Home Provides

Prime Home will guide and support you throughout the onboarding process and provide ongoing assistance to help ensure compliance with all state regulations and program requirements. Our team is dedicated to helping providers succeed while delivering quality care and support to the individuals we serve.

Application Process

Because the safety and well-being of individuals receiving services is our highest priority, the application process is comprehensive and includes:

1. Submission of a complete and accurate application.
2. Completion of phone and in-person interviews.
3. A home assessment and walkthrough of the residence where services will be provided.
4. Required background screenings for all applicants (Shared Living Providers, Alternative Care providers, and household members age 13 and older), including:
   • Criminal history checks
   • Adult Protective Services (APS) checks
   • Child Protective Services (CPS) checks
   • Sex offender registry checks
5. Completion of all required training programs.
6. Verification of current automobile insurance and homeowner's or renter's insurance.
