Job summary

The Nesconset Center for Nursing & Rehabilitation is seeking an Experienced Physical Therapist (PT) for our facility. Competitive hourly compensation up to $55 based on experience and position. Full-time with ongoing weekend shifts.

The PT will assess patients, establish treatment plans, document progress, and educate families while collaborating with the care team to deliver patient-centered rehabilitation services.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must have a valid license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care or geriatric care preferred.
• Strong documentation writing skills.
• Evaluations, weekly progress notes, recertifications and discharge documents.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service and clinical excellence.
• Be punctual and professional.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Document and educate patients and families in physical therapy methods.
