Job summary

The Nesconset Center for Nursing & Rehabilitation is seeking an experienced Physical Therapist (PT) to join our facility. The position offers compensation up to $55 an hour, based on experience.

You will assess patients requiring rehabilitation, establish and administer treatment plans, document progress, and educate patients and families. Experience with long-term care or geriatric settings, and supervision of therapy assistants, is preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Valid PT license/registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation and reporting skills.
• Demonstrates professionalism and punctuality.

Responsibilities

• Assess referred patients and provide direct rehabilitation care.
• Establish and manage individualized treatment plans.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
• Collaborate with other disciplines for patient-centered care.
