Nesconset Center for Nursing & Rehabilitation is seeking an Experienced Physical Therapist for our facility in Bayport, NY. The role offers an hourly rate “Up to $55 an hour” based on experience and position, with opportunities for professional growth and a stable work schedule.

The Physical Therapist will assess patients, design and monitor rehab plans, supervise PT Assistants, and coordinate with interdisciplinary teams to deliver high-quality care.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid professional license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.
• Conduct evaluations, progress notes, recertification and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Be punctual and professional; adhere to scheduled hours.
• Perform duties assigned by Director of Rehabilitation.
• Compliance with HIPAA and confidentiality.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with specific goals.
• Educate patients and families on physical therapy methods.
