Job summary

Mozaic Senior Life in Connecticut is seeking an Experienced MDS Coordinator to manage all MDS processes in a long-term care setting. You will work with nurse managers and the IDT to keep care plans aligned with residents' evolving conditions and ensure Medicare residents receive appropriate skilled services.

Requirements include RN licensure in Connecticut, CPR certification, and MDS/care-planning experience; BSN is preferred.

Qualifications

• Graduate of a school of professional nursing; BSN preferred.
• Strong clinical nursing background with MDS and care planning experience.

Responsibilities

• Oversee all MDS activities: initial admission, quarterly reviews, change of condition, Medicare and managed Medicare in Long Term Care.
• Collaborate with Nurse managers and IDT to ensure care plans reflect residents' conditions.
• Ensure Medicare residents are skilled.

Responsible for all MDS's including the initial admission, quarterly, change of condition, Medicare and managed Medicare in Long Term Care. Works collaboratively with Nurse managers and IDT to ensure that Care plans reflect the residents' condition on an ongoing basis. Ensure that all Medicare residents are skilled.

Graduate of a school of professional nursing; BSN preferred.

Strong clinical background in nursing; Head nurse, Care planning and MDS experience and fluency

RN State of Connecticut, CPR certification/IV-line management 3.0 certification.
