The role requires a Texas LVN license, hospice experience, and reliable transportation. You will participate in on-call rotations, IDG meetings, and maintain timely documentation.

Qualifications

• Current Texas LVN license in good standing.
• Minimum one year of hospice experience.
• Valid driver's license with reliable transportation.
• Ability to work independently and manage a daily patient schedule.
• Strong communication and documentation skills.

Mi Casa Hospice is seeking a compassionate, dependable, and motivated Licensed Vocational Nurse (LVN) to join our growing hospice team. This position provides direct patient care to hospice patients in their homes, assisted living facilities, and nursing facilities while working closely with the interdisciplinary team to ensure quality end‑of‑life care.

Responsibilities

• Provide skilled nursing visits to hospice patients according to the plan of care.
• Assess and document patient condition, symptoms, and changes in status.
