Los Altos Post-Acute is seeking a Case Manager to coordinate rehabilitation services with facility staff. The role monitors cost-effectiveness of treatments, facilitates admission and discharge, and acts as a liaison to insurance carriers and medical management professionals.

The ideal candidate holds a CA LVN or RN license, and has 3+ years in a health care setting with strong communication skills. The position emphasizes collaborative teamwork, timely task completion, and patient advocacy

Formación

• Active California LVN or RN license in good standing.
• Minimum 3 years experience in a hospital, nursing, or related health care facility.
• Working knowledge of managed care rules, regulations, and guidelines.
• Outstanding communication (oral and written) skills.
• Ability to multi-task and treat everyone at work kindly and professionally.
• Works collaboratively and independently, following procedures and guidelines.
• Completes tasks on time, reporting on successes, challenges and offering solutions.
• Emotional maturity - ability to remain calm and professional under stress.

Responsabilidades

• Greet new patients/residents ensuring a positive admission experience.
• Evaluate and communicate resident status and care plans to payers.
• Document all payer interactions regarding resident progress and instructions.
• Meet with interdisciplinary team to coordinate services ensuring total care regimen.
• Coordinate with business office and medical records for census and payments.

Case Manager

Position Summary

Coordinates the delivery of rehabilitation services to patients in collaboration with the facility staff.
