Job summary

Mountain-View Healthcare Center in Mountain View, CA seeks a licensed LVN/RN to coordinate rehabilitation services and advocate for patients throughout admission and discharge. You will collaborate with facility staff and payers to ensure accurate documentation and seamless transitions.

Qualifications

  • California LVN or RN license in good standing.
  • Minimum 3 years experience in a hospital or related health care facility.
  • Working knowledge of managed care rules, regulations, and guidelines.

  • Ability to multi-task and maintain professionalism.
  • Collaborates with team and adheres to procedures and guidelines.
  • Capable of reporting outcomes and offering solutions.
  • Remains calm and professional under stress.

Responsibilities

  • Greet new patients/residents ensuring a positive admission experience.
  • Evaluate and communicate resident status, changes in care plans to payers.
  • Document all payer interactions regarding resident progress and instructions.
  • Meet with interdisciplinary team to coordinate services and maintain the total care regimen.
  • Coordinate with business office and medical records to ensure census accuracy and payment for residents.

Job description

Coordinates the delivery of rehabilitation services to patients in collaboration with the facility staff. Continually monitors and documents the cost effectiveness of treatments provided, facilitating admission and discharge processes, serving as patient advocate and liaison to insurance carriers and other medical management professionals.

Qualifications:
  • Current California LVN or RN license in good standing
