Job summary

Elliot Health System in Manchester, NH seeks a Cardiac Rehabilitation Specialist to develop and implement education, exercise plans, and prescriptions for adult patients in the Cardiac Rehabilitation Program.

Responsibilities include leading group classes and individual counseling, conducting intake interviews, and tailoring risk-factor management. A BS in a relevant field and BLS certification are required, with prior cardiac rehab experience preferred.

Qualifications

• Bachelor's degree or higher in a relevant field.
• BLS certification required or obtainable at hire.

Responsibilities

• Provide education on cardiac and vascular disease and risk factor management through classes and individual sessions.
• Conduct intake interviews for outpatient cardiac rehabilitation and explain rehabilitation benefits.
• Assess patients and tailor risk-factor management based on history and labs.
• Manage and supervise exercise programs including warm-up, cardio, resistance training, and cooldown.
• Document assessments and interventions in the medical record in a timely manner.
• Support and contribute to overall program goals.

About the Job:

Responsible for developing and implementing the education, exercise plan and prescription for adult-sized adolescent and adult participating in the Elliot Health System Cardiac Rehabilitation Program following the standards as set forth by the American Association of Cardiovascular and Pulmonary Rehab (AACVPR).

What You’ll Do:

• Provides education on cardiac and vascular disease, risk factor management and healthy lifestyle modifications through group education classes and individual counselling sessions.
• Conducts interview/intake session with new patients for Outpatient Cardiac Rehabilitation, articulating benefits of the cardiac rehabilitation.
• Determines patient relative to risk factor management based upon the interview, pertinent laboratory, and medical history information.
• Conducts an exercise program which is appropriate for a cardiac and vascular population which will include, but not be limited to: warm-up, stretching, range of motion, cardiovascular conditioning, cool down activities, and resistance training; implements modification of exercise when patient’s physical or medical condition requires.
• Completes timely documentation of assessments, interventions, and evaluations of the Cardiovascular Health patient.

• Bachelor of Science in exercise science, exercise physiology, physical therapy or physical education, Required

• Prior experience in clinical cardiac rehabilitation or equivalent, Required
