Job summary

Oak Street Health, CVS Health company, is seeking an Executive Director, Care Solutions to lead end-to-end strategic oversight of integrated care programs within Population Health. You will shape partnerships, drive behavioral health innovations, and scale risk-bearing arrangements to improve value for patients and payers.

In this high-visibility role, you will manage a director-level team, collaborate with CVS Enterprise, Legal, Data and Finance, and establish KPIs to monitor engagement,

Qualifications

• 10+ years in Value-Based Care or Population Health.
• Proven track record negotiating multi-million-dollar risk-bearing contracts.
• Deep expertise in Behavioral Health integration (CoCM, PCBH).
• Strong financial acumen with direct exposure to P&L management.
• Experience navigating matrixed healthcare organizations and engaging executives.

Responsibilities

• Oversee strategic partnerships and multi-million-dollar contracts.
• Lead evolution from CoCM toward PCBH integrated care models.
• Direct daily partnership operations across cross-functional teams.
• Set and monitor KPIs for engagement, enrollment and outcomes.
• Collaborate with Legal, Data and Finance for quarterly reconciliations.
• Provide reporting to executive leadership on program performance.
