Job summary

The Center for Nursing and Rehabilitation at Hoosick Falls is seeking an experienced RN to join our team as a full-time evening shift supervisor (2pm-10pm) with weekend options. Candidates must hold an active NY RN license and have prior nursing home experience.

The role involves directing day-to-day nursing care, ensuring staff follow job descriptions, and providing leadership across shifts, admissions, rounds, and quality assurance activities within the facility.

Qualifications

• Must hold an active NY RN license.
• Prior nursing home experience required.
• Demonstrated leadership and supervisory capabilities.

Responsibilities

• Direct day-to-day functions of nursing care in accordance with current rules and guidelines for long-term care facilities.
• Ensure nursing service personnel follow their job descriptions.
• Assist in admitting, transferring, and discharging residents as necessary.
• Greet and assess all new admissions.
• Monitor and report accident/incident events to appropriate staff per policy.
• Provide leadership including administrative and supervisory principles.
• Lead staff meetings and in-services as required.
• Review resident care plans and ensure appropriate care is rendered.
• Review grievances and report actions to DNS or Social Service as needed.
• Review nursing documentation for accuracy and completeness; monitor changes in condition and rounds with physicians as necessary.
• Participate in government inspections and provide written/oral reports as required.
• Perform administrative duties such as charting and QA related tasks.
