Job summary

Community Care Center in Duarte, CA is seeking a dedicated staff member to oversee and monitor the safety of the facility, including crisis intervention and ensuring residents' wellbeing. The role requires watching for changes in condition, communicating with the Charge Nurse and Nursing Supervisor, and participating in in-service programs.

The position reports to the Program Director and requires a High School Diploma plus at least 2 years of experience working with the mentally ill.

Qualifications

Responsibilities

• Monitor and ensure safety of residents and the facility as assigned.
• Provide crisis intervention and assistance as needed.
• Observe and communicate changes in condition to the Charge Nurse or Nursing Supervisor or Program Manager.
• Conduct Line Of Sight Monitoring and 1:1 monitoring as needed.
• Assist with ADL's and documentation in PCC.
• Monitor floors and hallways and follow safety, infection control and Residents’ Rights policies.
• Work evenings or weekends as required.
