Job summary

Qualifications

• Two years of college education or a high school diploma/equivalent.
• At least 2 years of direct care work experience.
• Proficient in Microsoft 365.
• Experience navigating an electronic health records (EHR) system.
• Valid driver’s license and insurability.
• Ability to obtain CPR/AED/First Aid training.

Responsibilities

• Assist residents with independent living skills, including meal planning, cleaning, hygiene, and money management.
• Support clients in developing and following recovery plans, coordinating with treatment teams and natural supports.
• Provide advocacy, referrals, and connections to community resources.
• Facilitate rehab groups and interventions focused on skill building and recovery.
• Supervise self-medication protocols and ensure accurate documentation.
• Provide crisis intervention and collaborate with emergency services as needed.
• Maintain clear communication through progress notes, logs, and required records.
• Ensure a safe and supportive environment, including overnight coverage when assigned.
• Provide transportation and assist with scheduling personal appointments.
