• Provide direct patient care: Assist with activities of daily living (ADLs), administer medications, and record vital signs to ensure the health and well‑being of our residents.


• Monitor patient conditions: Report any changes in patient status to the Registered Nurse (RN) or physician and assist with implementing care plans as needed.


• Collaborate with healthcare team: Work closely with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.


• Maintain accurate records: Accurately document patient information, medications, and vital signs in accordance with facility policies and procedures.


• Participate in quality improvement initiatives: Contribute to improving patient outcomes and facility processes through active participation in quality improvement activities.
