2nd Shift RN Case Manager – Hospice Team, Peoria (Monday–Friday, 8‑hour second shift)
Job Overview
The Care Team, in partnership with Traditions Health, seeks a qualified registered nurse to provide case management and skilled nursing care to home health patients during the second shift. Compensation ranges from $77,550.00 to $91,650.00, accompanied by a comprehensive benefits package.
Job Responsibilities
- Conduct initial and ongoing assessments (e.g., OASIS) to identify patients’ physical, psychosocial, and environmental needs.
- Develop, initiate, and revise individualized plans of care to ensure quality and continuity of service.
- Implement preventive and rehabilitative nursing procedures, refer patients to other services, and coordinate discharge planning.
- Provide nursing care that requires substantial and specialized skill; counsel patients and families on nursing and related needs.
- Maintain infection control practices and communicate changes in patient status to physicians and interdisciplinary team.
- Ensure documentation, including clinical and progress notes, meets third‑party requirements and is completed in a timely manner.
- Supervise, teach, and provide clinical direction to other nursing personnel and home health aides.
- Perform on‑call duties as required.
- Maintain professional growth by participating in in‑service programs and continuing education.
Qualifications & Requirements
- Schedule: Monday‑Friday, second shift (8‑hour shifts)
- Education: Graduate of an accredited diploma, associate, or baccalaureate school of nursing
- Licensure: Current state license as a registered nurse; valid driver’s license and auto insurance
- Experience: Minimum one year of clinical nursing experience; home health experience preferred
- Skills: Proficient nursing skills, good interpersonal communication, CPR certification, ability to lift up to 50 lbs, and navigate flexible schedules
- Transportation: Reliable personal vehicle and valid driver’s license
Working Conditions
Work is performed in patients’ homes in varying conditions, with exposure to blood, bodily fluids, and infectious diseases. The role requires flexibility in scheduling, local travel, and occasional work in unpleasant weather. Physical demands include prolonged standing, walking, and lifting up to 50 lbs.
