Job summary

LiveWell Homecare Agency, LLC seeks an RN Coordinator of Patient Care Services to lead daily unit operations and model clinical excellence in patient care. The role evaluates patient needs, plans staffing and flow, supports staff development, and partners with interdisciplinary teams to drive quality and safety across the care continuum.

Evening shifts are required with physical demands including lifting and patient transport.

Qualifications

• Diploma in nursing and active RN license.
• At least 3 years of acute care RN experience required.
• Strong communication skills and cultural awareness.
• Knowledge of care coordination, patient safety and documentation.

Responsibilities

• Lead daily unit operations and ensure patient flow.
• Evaluate patient needs, make care recommendations, and ensure staffing.
• Serve as a resource for staff and collaborate with interdisciplinary teams.
• Support nursing professional development and shared governance.
• Monitor staff schedules and address patient and staff issues.
• Participate in quality improvement initiatives.
