Job summary

CODA is seeking a dedicated care assistant in Portland, Oregon to support patients in a structured, therapeutic community. You will follow a Monday–Friday 2pm–10:30pm schedule, monitor patient behavior, document routine observations, and respond to medical needs per site procedures.

You will assist with outings, help with medication administration, perform safety checks, document in the EMR, maintain confidentiality, and contribute to a respectful, recovery-focused environment.

Qualifications

• Must have high school diploma or GED.
• Experience with clinical documentation and compliance is preferred.
• Ability to follow site procedures and safety protocols.

Responsibilities

• Schedule: Monday–Friday 2pm–10:30pm.
• Monitor patient behavior and location; document routine observations.
• Respond to emergencies and medical needs per site procedures.
• Assist with self-administration of medications and record in the medication log.
• Conduct safety checks of the grounds and prevent contraband.
• Maintain patient confidentiality in accordance with HIPAA and 42-CFR.
