Responsibilities

• Administer medications and treatments as prescribed by the physician.
• Monitor, assess, and document resident condition, progress, and response to treatments.
• Assist in implementing individualized care plans with rehab and nursing teams.
• Communicate with residents, families, and interdisciplinary team members.
• Respond promptly to resident needs and emergencies.
• Ensure compliance with facility policies and regulatory requirements.
• Provide support to rehab aides and other team members.
