Job summary

Qualifications

Active, unrestricted license in PA.
Doctor of Pharmacy degree is required.
Experience with compounding infused drugs.
Proven record of evaluating and monitoring drug therapies for home care patients.
Excellent verbal and written communication skills.
Shift: 3:30pm–12:00am, Mon-Fri with alternating weekends.

Responsibilities

Accept and dispense prescription medications.
Monitor patient medication compliance and over-usage patterns.
Ensuring that the supply of medicines is within the law.
Ensuring that the medicines prescribed to patients are suitable.
Maintain confidential patient records and pharmacy files.
Advise patients about medicines, dosing, and potential reactions.
Provide counseling on prescription and non-prescription medications.
Provide services such as smoking cessation, BP measurement, and cholesterol management.
Educate patients on proper handling of medical devices.
Compound medications for external or internal use.
Prepare sterile solutions following proper procedures.
Supervise pharmacy staff.
Maintain inventory of pharmaceutical supplies.
Identify drug-related problems and communicate with physicians.
