Job summary

Qualifications

Responsibilities

• Provide and collaborate on direct patient care under supervision, including ADLs, vital signs, and care plan implementation.
• Observe and report changes in patient condition to a licensed caregiver and document care delivered.
• Address questions or concerns from patients, families, or health care team members.
• Perform other duties as assigned.
