Trilogy Health Services, LLC is seeking an Evening Supervisor (LPN/RN) to assist the DON/ADON in overseeing the nursing department’s evening operations at The Springs at Stony Brook in Louisville, KY. The role focuses on maintaining the highest quality of care while guiding staff and ensuring compliance with patient plans.

The position requires a current state LPN or RN license and CPR certification, with 8-hour evening shifts, Monday–Friday, and on-call rotation.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Current CPR certification required.

Responsibilities

• Assist DON and ADON in planning, organizing and directing the nursing service department in the evening shift.
• Supervise and assist with resident care according to plan of care and assessments.
• Provide direct nursing care as necessary and assess treatment results.
• Support documentation processes in MatrixCare to ensure records are complete and accurate.
• Help maintain campus readiness for tours, surveys, and model units.
• Provide patient care as needed.
