Oakmont Management Group’s Ivy Park at Pleasanton is seeking a Medication Nurse (LVN/LPN) for on-call/part-time PM shift (2pm–10pm) in Pleasanton, CA. The role administers medications per physician orders and maintains medication records.

Qualifications

• Must hold a valid California nursing license (LVN/LPN).
• Prefer 1 year of experience assisting with medications in RCFE setting.
• Ability to work with seniors and cognitively impaired individuals.

Responsibilities

• Administer medications as ordered by physician.
• Supervise medications room/cart and maintain accurate records.
• Coordinate medication services with residents, pharmacy, and physician.
• Monitor psychotropic medications per physician orders and document changes.
• Maintain confidentiality of resident medication records and ensure documentation is current.
