Oakmont Senior Living in San Jose is seeking a Medication Nurse (LVN/LPN) to supervise and administer medications to residents under physician orders. You will maintain medication records, ensure compliance with policies, and coordinate with pharmacy and care teams.

The role requires CA license (LVN/LPN) and is oriented toward high-quality elder care, with shifts Tue-Sat 2:00pm-10:00pm; benefits include medical, dental, vision, paid time off, and more.

Qualifications

• Must hold a valid California LVN/LPN license in good standing.
• Prefer 1 year experience with medications in RCFE or elder care.
• Knowledge of assisted living and dementia care principles.

Responsibilities

• Administer prescribed medications under physician orders.
• Supervise medication room, maintain accurate med records.
• Coordinate with residents, pharmacy, and physicians.
• Prepare medications for distribution and assist with administration.
• Document refusals, changes, and dosage updates per policy.
• Maintain organized, stocked medication areas and secure access.
• Observe and report any discrepancies or safety concerns.
