Job summary

Trellis Chino is seeking a Licensed Vocational Nurse (LVN) for the 3:00PM–11:00PM shift. The role features a 4 days on / 2 days off rotation, serving a 59-bed skilled nursing facility near Chino Valley Medical Center.

The pay range is $29.00–$33.00 per hour, DOE, with benefits for full-time staff. Responsibilities include administering medications, documenting care in EMR, monitoring resident conditions, supervising CNAs, and collaborating with physicians and the care team to deliver

Qualifications

• California LVN license in good standing.

Responsibilities

• Administer medications and treatments per physician orders.
• Document patient care in EMR.
• Monitor resident conditions and report changes.
• Provide care for residents with acute and chronic needs.
• Supervise and support CNAs.
• Communicate with residents, families, physicians, and interdisciplinary team members.
• Utilize critical thinking to make sound clinical decisions.
• Ensure compliance with facility policies, procedures, and regulatory requirements.
