Job summary

Community Extended Care Hospital of Montclair is seeking an LVN to join our skilled nursing team. The LVN will provide direct nursing care to residents and work closely with CNAs, RNs, physicians, and other team members in a resident-centered environment.

The ideal candidate will have a California LVN license, BLS certification, strong clinical skills, and a compassionate approach. Evening shifts are available, with competitive hourly pay and comprehensive benefits.

Qualifications

• Active California LVN license is required.
• BLS certification is required.
• Experience in SNF, skilled nursing, or geriatric care is preferred.
• Strong clinical assessment and documentation skills are essential.

Responsibilities

• Provide nursing care per physician orders and care plans.
• Administer medications and treatments safely.
• Monitor residents’ condition, vitals, and responses.
• Document changes and communicate with the care team.
• Supervise CNAs and other staff as assigned.
• Participate in care plan meetings and documentation.
