The Willows of Ramsey Hill, part of Episcopal Homes, seeks an LPN for the evening shift at 80 North Mackubin Street, St. Paul, MN 55102. Serve in a 58-bed memory-care setting, delivering meds, vital signs, and care plans under supervision.

Join a resident-centered team dedicated to Alzheimer’s and dementia care. You’ll collaborate with physicians and caregivers, document health status in EMR, and participate in infection control and quality initiatives.

Qualifications

• Licensed Practical Nurse in good standing with the Minnesota Board of Nursing.
• CPR and First Aid certification.
• Experience with EMR systems preferred (Eldermark).

Responsibilities

• Assist in developing individualized care plans for residents
• Provide direct nursing care including medication administration, monitoring vitals, and routine treatments
• Operate and monitor medical equipment to assess resident health
• Observe residents and document health status, changes, and treatment responses
• Supervise and support caregiving staff to ensure adherence to care plans and protocols
• Advocate for residents by educating families, caregivers, and residents on care and wellness strategies
• Communicate resident condition changes to appropriate staff, providers, and families
• Collaborate with physicians, therapists, and healthcare professionals to ensure quality care
• Maintain accurate and timely nursing documentation in EMR systems
• Participate in infection control, quality assurance, and compliance initiatives
