Job summary

Bond Health in Queens, NY is seeking a Licensed Practical Nurse (LPN) for a part-time role at our Group Home. You will supervise staff, administer medications, monitor health conditions and maintain safety and regulatory compliance.

The position requires a NY LPN license, CPR certification, and good communication. Experience with individuals with intellectual and developmental disabilities preferred. Evening shift 7:00 PM–7:00 AM on Tue, Fri, Sat and Sun with some training and appointments.

Qualifications

• Current unrestricted New York State LPN license.
• CPR certification (AHA or equivalent).
• Experience with individuals with intellectual and developmental disabilities preferred but not required.
• Excellent written and verbal communication skills.
• Ability to work independently and as part of a team.
• Valid New York State driver’s license highly preferred.

Responsibilities

• Supervise and assist direct support staff with personal care and daily living activities.
• Administer medications according to established procedures.
• Monitor infection control practices and maintain first aid supplies.
• Document medical findings and report significant changes to the RN Supervisor.
• Communicate with healthcare providers and family members regarding medical concerns.
• Provide treatments including wound care, G-tube care, blood glucose monitoring, urinary drainage maintenance, and ROM therapy as ordered.
• Monitor vital signs, weights, and other health indicators.
• Ensure compliance with federal and state regulations.
• Attend medical appointments and required training.
• Perform other duties as assigned.
