Job summary

Presbyterian SeniorCare Network seeks an LPN for Manchester Commons in Erie, PA, working evenings on a full-time, hourly non-exempt schedule. The role focuses on coordinating and supporting personalized care plans for residents within a unified facility, including dementia care pathways and supervision of care staff.

Eligible candidates hold a PA LPN license, with preferred experience in personal care or assisted living, and CPR certification.

Qualifications

• Current, valid Practical Nurse License in good standing from the state of Pennsylvania.
• Experience as a licensed nurse preferred.
• Experience in Personal Care or Assisted Living preferred.
• Knowledge of DHS regulations preferred.
• Dementia care experience preferred.
• Maintains CPR certification.

Responsibilities

• Coordinate and support individualized service plans for residents.
• Provide leadership and direction to community team members to ensure quality of care and services.
