Job summary

Crestview Home is seeking a dedicated Licensed Practical Nurse (LPN) for the Evening shift in Bethany, MO. You will provide direct patient care, administer medications, and help build individualized care plans in collaboration with the interdisciplinary team.

Qualifications

• LPN licensure in Missouri (required).
• High school diploma or equivalent required.
• BLS certification preferred.

Responsibilities

• Provide direct patient care to residents, including administration of medications and wound care.
• Develop and implement individualized care plans with the team.
• Document patient information accurately during assessments.
• Assist with activities of daily living (bathing, dressing, grooming).
• Communicate with residents, families, and healthcare team for seamless care.
• Participate in quality improvement initiatives to improve outcomes and satisfaction.
